Description
Use this Circle Time Data Collection Form to track your student’s progress during large group time. The form is editable to fill in the date, plus or minus, number of prompts, activity/task, and comments. There are three forms included. Two forms that have behaviors already filled in to track and one form that is blank so you can fill in your own.
Directions:
1. Editable circle time data sheet to use in your classroom based on your student’s IEP Goals. You can print this and put in a binder or keep as a digital form.
2. These data sheets are to track IEP goals or behavior goals and progress over time.
3. In my classroom, I put the student’s goals and data sheets into a binder for each student. That way I have quick access to all of their individual goals and data. This is useful when I write my IEP reports and progress reviews.
